Hazardous Waste Reporting

Notification and Identification

EPA Form 8700-12 - Notification of RCRA Subtitle C Activities: Used to notify the EPA of a facility's hazardous waste activities, including obtaining an EPA Identification Number and declaring generator status (e.g., Large Quantity Generator, Small Quantity Generator, or Very Small Quantity Generator).

Biennial Hazardous Waste Report

EPA Form 8700-13A/B - Biennial Report for Large Quantity Generators (LQGs): Required every two years, this report provides detailed information on the types and quantities of hazardous waste generated, managed, and disposed of by facilities classified as Large Quantity Generators (LQGs). This includes two main parts:
- Form 8700-13A: Site Identification Form
- Form 8700-13B: Waste Generation and Management Report

Manifest Forms

EPA Form 8700-22 - Uniform Hazardous Waste Manifest: Used to track hazardous waste from the point of generation to the final disposal or treatment site. This document includes information on the type and quantity of waste and is signed by each handler in the waste management chain.

Exception and Discrepancy Reports

Exception Report: Mostardi Platt can assist you in the event that your facility needs an exception report. The report is required when a generator does not receive a copy of the hazardous water manifest signed by the designated facility within a specific time frame (typically 35 days for LQGs or 60 days for SQGs). Discrepancy Report: A report may need to be filed when there is a significant discrepancy between the waste description or quantity on the manifest and what is received at the designated facility.
